Forms

On this page, you will find links to MCJC forms. It is a work in progress and will eventually hold all forms that MCJC produces for congregational use. Forms are fillable, that is, you can download them, save to your files, open and fill them in if your system allows it.

Completed forms can be returned in three ways:

  • After completing a fillable form and saving it in your files, you can attach it to an email and email it to forms@mcjconline.org.
  • You can print the form, fill it in either on your computer or manually, take a phone picture of the completed form, and email the picture as an attachment to forms@mcjconline.org.
  • You can print the form, fill it in either on your computer or manually, and mail the completed form to MCJC, 8610 Ridgefield Rd., Crystal Lake, IL 60012.

CALENDAR AND EVENT PROMOTION FORM

Instructions for the CALENDAR AND EVENT PROMOTION FORM:

To submit dates to the MCJC Calendar for promotion in the e-nouncements or print newsletter, for press releases or any other kind of promotion, please use the CALENDAR AND EVENT PROMOTION FORM available on the MCJC office door or online at https://mcjconline.org/forms/.

When you have completed your form — a fillable .pdf — attach it to an email and send to forms@mcjconline.org. You can also fill it in manually and mail or drop off to the synagogue office (8617 Ridgefield Rd., Crystal Lake, IL 60012) marked Attn. CALENDAR & EVENT PROMOTION FORM.

All events and dates must be BOARD APPROVED before editing, calendar entry, and posting in our various venues can begin. Leave plenty of time to meet publication deadlines.

Deadline for the January/February Newsletter, MCJC Connections, has passed. Next Newsletter is March/April, and the deadline is February 1. Your submissions must be BOARD APPROVED by that date. Again, leave plenty of time.

Once your request is approved, if you have additional or updated copy or pictures, email to communications@mcjconline.org. Pictures should be in .jpg format.

If you have specific questions about the e-nouncements or are not receiving newsletters or e-nouncements, please notify the Communications Committee via communications@mcjconline.org.
